Sciatica refers to pain that travels along the path of the sciatic nerve, which runs from your lower back through your buttock and down the back of your leg. It’s typically caused by compression or irritation of nerve roots in the lumbar spine, most commonly from a herniated disc, though spinal stenosis, bone spurs, or other conditions can also be responsible. The pain often shoots down one leg and may be accompanied by numbness, tingling, or weakness. True sciatica follows a specific nerve distribution, though the term is sometimes used more broadly for any leg pain originating from the back.
Most acute sciatica episodes improve within six to twelve weeks with conservative treatment, as the body’s inflammatory response settles and disc herniations often reduce in size naturally. However, duration varies considerably. Some cases resolve within weeks, whilst others persist for months or become chronic. Factors affecting recovery include the underlying cause, severity of nerve compression, and your overall health. If symptoms haven’t improved after six to eight weeks of appropriate conservative treatment, or if you have progressive weakness, further investigation and consideration of more active interventions like injections or surgery may be warranted.
Prolonged sitting often aggravates sciatica, particularly in soft seats that cause you to slump. Heavy lifting, especially with poor technique, can worsen nerve compression. High-impact activities like running on hard surfaces might increase symptoms. However, complete rest is actually counterproductive. Gentle movement and walking within tolerance help more than strict bed rest. Avoid movements that significantly increase your leg pain, but staying generally active promotes healing. Each person’s tolerance differs, so pay attention to what aggravates your symptoms. Activities that centralise pain from your leg towards your back are usually safe, whilst those that peripheralise pain into the leg should be avoided.
Many people recover completely from sciatica episodes and have no further problems. Once the acute herniation heals and inflammation settles, the nerve recovers. However, if you have underlying degenerative disc disease, there’s a risk of recurrence at the same or different levels. Maintaining good core strength, proper lifting technique, and healthy weight reduces this risk. Some people have recurring episodes requiring ongoing management. If a specific structural problem like a large disc herniation is causing persistent sciatica, surgery to remove the herniation can provide definitive relief, though it doesn’t prevent future disc problems elsewhere.
